Deep dissectio. A blunt retractor is placed beneath the muscle belly of the VMO, elevating it away from the medial side of the distal femur. The descending genicular vessels, accompanying veins, and vessels to vastus medialis should be identified and ligated or cauterized to avoid a post-operative hematoma.
